The fees listed below include the cost of the sessions and ongoing support between sessions.

Self-Pay Rates

Individual Appointments:

  • One (1) Comprehensive Initial Nutrition Assessment (up to 60 minutes) – $179
  • One (1) Personalized Follow-up Session (up to 45 minutes) – $119

Follow-up Packages:

  • Three (3) Personalized Follow-up Sessions (up to 45 minutes each) – $319 (11% savings)
  • Five (5) Personalized Follow-up Sessions (up to 45 minutes each) – $499 (16% savings)

Group Sessions and Lab Testing – COMING SOON

The initial assessments serve the purpose of thoroughly evaluating your medical history, dietary intake, blood sugar monitoring routines, and dietary preferences. In addition, we will engage in in-depth discussions covering a range of nutrition education topics, goal setting, and lifestyle adjustments, all geared towards empowering you to effectively manage your diabetes or other medical conditions.

As we progress, our follow-up sessions will be dedicated to tracking your advancements, making any necessary plan adjustments, and providing support throughout your journey. Our collective goal is to ensure that your health and wellness plan not only succeeds but thrives.

Insurance

Now accepting most United Healthcare, Aetna, Anthem, Blue Cross Blue Shield, and Medicare plans!

Understanding your insurance coverage can be complex, but we’re here to simplify the process. Upon submitting an application to work with us, you’ll receive an outline detailing the necessary information to obtain from your insurance company.

To ensure a seamless experience in utilizing your insurance benefits, please review the following guidelines:

  • Contact Your Insurance Provider: Before your appointment, it’s crucial to reach out to your insurance provider to confirm coverage for nutritional counseling. You can find the phone number for member services on the back of your insurance card.
  • CPT Codes: If asked for CPT codes, provide them with 97802 and 97803. If these codes are not covered, ask about 99401, 99402, 99403, and 99404.
  • Diagnosis Codes: If asked for a diagnosis code, provide Z71.3. If not accepted, consider Z72.4. Additionally, coverage may vary for conditions such as overweight, obesity, pre-diabetes, diabetes, hypertension, or high cholesterol.
  • Cost-Sharing: Be aware that a cost-share, including deductibles, co-pays, or co-insurance, may apply. We’ll bill under your preventive benefits if applicable, potentially eliminating your cost-share.
  • Specialist Co-pay: Since dietitians are often classified as specialists, your specialist co-pay may apply. This information is typically available on your insurance card, although due to our billing process, the co-pay may not always be applicable.
  • Direct Billing: Rest assured, we handle billing directly with your insurance provider. Any patient responsibility will be charged to the credit card on file once we receive the Explanation of Benefits (EOB).
